Applied Computing Student Forms

Students use various forms to make academic requests within the Division of Computing & Software Systems (CSS) such as petitioning a course or graduation requirement, requesting to repeat or take a CSS waitlisted course, enrolling in independent study or computing internship, participating in undergraduate research, requesting an incomplete grade, and submitting a capstone contract.

CSS Course & Graduation Petition

Use this form if you want to petition any course or g raduation requirements for the CSSE major, Applied Computing major, CSSE minor or IT minor. This petition also allows you to verify transfer course equivalency for a CSS course for admission or non-CSS degrees requirements (if requested by the program).

When to apply: Submit your petition as soon as possible for the following actions:

  • Graduation requirements: A quarter before you plan to take a course that is not part of the CSS division. This includes computer science courses at UW Tacoma and UW Seattle and other schools across the nation.
  • Course equivalency: If you are a current student and think a CSS course is equivalent to a course at UW Bothell, submit a syllabus (code and assignments may be request) as soon as possible. Course equivalency is subject to petition approval.
  • Prerequisite alternative for core classes: If you are UW Bothell non-major student who does not meet the prerequisites for a CSS core class then you must submit the CSS Course & Graduation Petition form.
  • Other actions: Please connect with cssadv@uw.edu if you are unsure if you should file this petition.

How to apply: Make sure you have a complete syllabus, a well thought out and a developed persuasive statement (educational reasoning and how this will help your CSS experience) and projects or related coursework that helps support your request.

Submit the CSS Course & Graduation Petition

Independent Study Contract: CSS 198 and CSS 498

CSS 198 (cannot count towards major CSS requirements) and CSS 498 are supervised courses that you create and propose to CSS faculty that give you the opportunity to learn about a subject that is unavailable in the CSS curriculum or on a topic that you would like to explore in greater depth. The individual quarter credit range can be 1-5 credits of CSS 198 (maximum of 6 credits) and of CSS 498 (maximum 10 credits).

When to apply: Once the contract has been agreed upon by the CSS faculty member you can be added to the course by an adviser when your registration for that quarter has opened.

How to apply: Submit the finalized contract to cssadv@uw.edu for an adviser to assist you. You must obtain the approval signature of a full-time CSS faculty member who will act as your instructor. This course ranges from 1-5 credits, but the number you enroll in will be at the discretion of the same CSS faculty member.

Computing Internship Contract: CSS 397 and CSS 495

This an opportunity for students to obtain college credit by participating in an internship course supervised by a full-time CSS faculty member in which the student meets the faculty’s objective while participating in an internship related to the field of computing.

When to apply: When you have an internship offer and have completed the contract with a full-time CSS faculty member who has agreed to serve as your faculty supervisor. This can be 1-5 credits (maximum 5 credits of CSS 397, maximum 10 credits CSS 495) and is determined by the CSS faculty member.

How to apply: Submit the finalized contract to cssadv@uw.edu when your registration has opened so an adviser can add you to the course.

Undergraduate Research Contract: CSS 199 and CSS 499

CSS 199 (cannot count towards major CSS requirements) and CSS 499 is an opportunity for students to obtain college credit by participating in a course that is supervised by a full-time CSS faculty member in which the student meets the faculty’s objective while participating in a faculty sponsored research project related to the field of computing.

When to apply: When you have completed the contract with a full-time CSS faculty member who has agreed to serve as your faculty supervisor. CSS 199 can be 1-5 credits (maximum 6 credits) and CSS 499 can be 0-5 credits (maximum 10 credits) and is determined by the CSS faculty member.

How to apply: Submit the finalized contract to cssadv@uw.edu once your registration period has opened so an adviser can add you to the course.

Incomplete Grade Contract

This is a contract for students who qualify by instructor permission for an incomplete grade and are working with a CSS faculty member to outline and set expectations of the “I” grade that the faculty has approved.

When and how to submit: After approved and confirmed by CSS faculty, you can email the contract to cssadv@uw.edu so we can also have a copy of the contract for reference. You are responsible for keeping a copy for yourself.
